President's Statement

Myrtle Hilliard Davis Comprehensive Health Centers, Inc. (formerly named St. Louis Comprehensive Health
Centers, Inc.) for over 18 years and serve in various positions within the company in the capacity of
Comptroller, V.
P. Business Affairs, and Chief Financial Officer.

It has been my strong commitment to strengthening the very basic of the health center primary care
infrastructure and
improving the quality of clinical services. In appreciation of your choice to select MHDCHC as your Primary Care
Provider (PCP), the health center and its’ Board of Directors remain committed in offering preventive and
comprehensive primary health care services that’s affordable and accessible regardless of patient’s inability to
pay.
We offer a sliding fee scale, which is based on income and family size.

The Health Center provides primary care services from three locations. Our main site is
located at 5471 Dr. Martin Luther King Dr. and our satellite sites are located at, Homer G. Phillips 2425 Whittier,
and Florence Hill 5541 Riverview. All sites have been certified as a Federally Qualify Health Center (FQHC).
With the assistance of Federal Grants from U.S. Dept of Health and Human Services
which has afforded us the opportunity to provide core services, which are Pediatrics,
Family Medicine, Adult Medicine, Women's Health, Podiatry, Ophthalmology, Dentistry,
Laboratory and Pharmacy, which are augmented with Enabling Services.

With our unique partnerships and collaborations within the community we have began to
embark on new frontier in providing preventive medicine and education to our patients.
The MHDCHC has changed its’ practice to the chronic care model that holds the patient
and provider accountable for better health outcomes. All services are available Monday
through Friday from 9:00am to 5:30pm with extended hours to 8:30pm at alternate sites.
